Enrollment Forms

Returning Students:

Returning students need to complete only the Update Form – click here.

New Students:

New students need to have the Enrollment, Contract and Discipline Forms on file in order to register.  Prior to the child’s first day of school we will also need the Health Form (completed by the child’s doctor) along with a current copy of immunizations.

-         Page four (Medical Release) needs to be notarized.  Our director is a notary and would be happy to notarize this for you.  Please wait until you are in our office to sign that page.

-         This form must be accompanied by a current immunization record before your child can start school.
