Enrollment Procedure / Forms for 2009-2010 School Year

PROCEDURE

In order to enroll your child in our program you will need to 1) pay the registration fee and 2) complete the Enrollment forms found below.  Please feel free to give our office a call to check on space availability - 972-398-7560.  If you have to leave a message, please let us know the date of birth of your child.  Click here to see our 2009-2010 Tuition Rates / Fees.

FORMS

Returning Students:

Returning students need to complete only the 2009-2010 Update Form – click here.

New Students:

New students need to have the Enrollment, Contract and Discipline Forms on file in order to register.  Prior to the child’s first day of school we will also need the Health Record (completed by the child’s doctor) along with a current copy of immunizations.

-         Before initialing page three, make sure you read your Parent Handbook.

-         Page four (Medical Release) needs to be notarized.  Our director is a notary and would be happy to notarize this for you.  Please wait until you are in our office to sign that page.

-         This form must be accompanied by a current immunization record before your child can start school.
